Certificates: how to get them
21Application for certificate
An application for a certificate must—
- be made to a Licensing Authority on a form approved by the chief executive of the Ministry of Justice after consultation with all Licensing Authorities; and
- be accompanied by a photograph of the applicant, authenticated in the manner required under section 77AA; and
-
- be accompanied by any prescribed fee.
The approved form must, without limitation, require the following information to be supplied:
- the applicant’s full name and date of birth:
- the applicant’s contact address and prescribed contact details:
- any other prescribed information.
On receipt of an application, a Licensing Authority may make whatever inquiries he or she considers necessary in order to determine whether or not the applicant is disqualified from holding a certificate.
