15Application requirements

  1. An application is made when the Area Director receives an application that complies with this section.

  2. An application must—

  3. state that the applicant is 18 years of age or over; and
    1. state that the applicant has personally seen the person whom the applicant seeks to have assessed within the 5 days immediately before the date the application is made; and
      1. state the grounds on which the applicant believes that the person whom the applicant seeks to have assessed has a severe substance addiction; and
        1. be dated and signed by the applicant; and
          1. be accompanied by—
            1. a medical certificate under section 17; or
              1. a memorandum under section 18.
