When a baby is born using a donor, the provider must tell the Registrar-General. You need to know the provider will try to get information about the baby, like the date and place of birth, the baby's sex, and the baby's name. The provider will then give this information to the Registrar-General.

The provider will also give the Registrar-General the names and addresses of the baby's guardians and information about the donor. This information about the donor includes their name, address, and date, place, and country of birth. The provider will use a special form to give the Registrar-General this information.

The provider must also tell the Registrar-General their own name when giving them the information about the baby and the donor. This is so the Registrar-General has a record of who provided the information. You can think of the Registrar-General like a record keeper for important information like this.
