If you are 18 years or older and were born using a donor, you can choose to let the donor know who you are. You can do this by giving a written notice to a provider or the Registrar-General, saying it is okay to share your identifying information with the donor. You can also change your mind later and cancel this notice. When you give this notice, the provider or the Registrar-General must keep it with your other information. The provider or the Registrar-General can only share your information with the donor if you have given them a notice saying it is okay, and you have not cancelled that notice.
