When you have a care and rehabilitation plan, it must include some important details about you. It must identify your social, cultural, and spiritual needs, as well as any medical or psychological treatment you require. It must also consider any medication you need to manage your condition.

Your plan must think about times when you might behave in a way that could endanger yourself or others. It should also look at your skills and aptitudes, and try to maintain and encourage them if possible. You might have special concerns or aversions, or special dietary needs, and these must be considered in your plan.

When identifying your needs, your plan must take into account any cultural assessment that was completed under section 23. Your plan must say how your needs can be met, and what kind of supervision you require to stay safe. It must make sure you get the right supervision to avoid any undue risk to your health or safety, or the health or safety of others.
