31Application to be accompanied by certificate

  1. Every application under section 29 must be accompanied by a certificate of a person who—

  2. is a medical practitioner or a psychologist; and
    1. has examined the person to be assessed within the 3 days immediately before the date of the application.
      1. Every certificate given under subsection (1) must state—

      2. that the person giving the certificate is a medical practitioner or a psychologist and has examined the person to be assessed, and the date of the examination; and
        1. that, in the opinion of the person giving the certificate, there are reasonable grounds for believing that the person to be assessed may have an intellectual disability; and
          1. the reasons for that opinion, explaining why the person giving the certificate believes that the condition of the person to be assessed may come within the definition of the term intellectual disability in section 7.
