38Application for licence

  1. An application for a licence under this Part must—

  2. be made to the Registrar in the prescribed form; and
    1. be accompanied by a declaration made in the form prescribed in the Oaths and Declarations Act 1957 that the person is eligible to obtain a licence by virtue of section 36 and is not prohibited from being licensed under section 37; and
      1. be accompanied by the prescribed fee (if any).
