86Contents of name-change certificate

  1. A name-change certificate in respect of a person must contain the information—

  2. that is recorded in the registry in relation to the person; and
    1. that is required by regulations to be contained in a name-change certificate.
      1. A name-change certificate may be issued in respect of a person only if the person’s birth is not registered under this Act.
