You can make this request or your personal representative can make it for you. Your representative must have your written permission to make the request. 99Individual or individual’s representative may request copy of access register

  1. The following persons may request the Registrar-General to provide a copy of all entries made in the access register in relation to information in the registry about an individual’s birth, death, marriage (or intended marriage), civil union, or name change:

  2. the individual who is the subject of the information:
    1. the individual’s personal representative.
      1. The request must—

      2. be made in accordance with any directions issued by the Registrar-General; and
        1. be accompanied by—
          1. the prescribed fee (if any); and
            1. any means of identification that the Registrar-General reasonably requires to confirm the identity of the person making the request.
            2. An individual’s personal representative who makes an application on behalf of the individual under subsection (1)(b) must have the written authority of, or be otherwise properly authorised by, the individual to make the request.
