111Applications for accreditation

  1. Every application for accreditation under section 109 must—

  2. be made in writing to the Director-General; and
    1. be made on a form provided by the Director-General and in a manner approved by the Director-General; and
      1. be accompanied by the prescribed fee (if any).
        1. For the purpose of assessing the matters specified in section 109(2), the Director-General may require an applicant to supply information additional to that contained in the application.

        2. If the applicant fails to supply the information within 3 months after the request, or within such further time as the Director-General may allow, the application lapses.
