If you want to ask for a formula assessment for child support, you need to fill out a special form. This form will ask you for some important information.

You need to tell us about at least one child who needs support. For each child, you need to name at least one person who takes care of them for at least 35% of the time. You also need to name at least one parent who might have to pay child support.

The form will ask for each child's tax file number. This is a special number the government uses to keep track of money. If you don't know this number, the form might let you leave it out.

You have to make sure everything you write on the form is true. You might need to add some extra papers with the form. These papers need to be true too.

10Form of application

  1. An application for formula assessment must be in an approved form and be accompanied by the documents (if any) specified in the form.

  2. Every application must—

  3. identify at least 1 qualifying child to whom the application relates; and
    1. identify, in relation to each qualifying child, at least 1 person who provides at least 35% of ongoing daily care to the child; and
      1. identify at least 1 person as a liable, or potentially liable, parent of each qualifying child identified; and
        1. include the tax file number (as defined in section YA 1 of the Income Tax Act 2007) of each qualifying child (except to the extent that the application form permits otherwise).
          1. The application, and every document accompanying it, must be verified as specified in the application form.

          2. An application for a formula assessment is properly completed if—

          3. it contains all the information required by the application form to be supplied; and
            1. it is accompanied by all the documents required by the application form to accompany the application; and
              1. the application and documents are verified as required by the application form.
