216CForm of request for transfer of refund

  1. A request made under section 216B must—

  2. be in writing; and
    1. specify—
      1. the amount of the refund that is to be transferred; and
        1. the tax type or financial support in relation to which the refund is to be transferred; and
          1. the name of the person who is to be credited with the amount (if that person is not the person who is making the request); and
            1. any other information that the Commissioner may require.
