138Form of application

  1. An application for a special licence—

  2. must be made in the name of the person who will hold it if the application is granted; and
    1. must state whether the applicant wishes the licence to be designated as an on-site special licence, an off-site special licence, or both; and
      1. must be made in the prescribed form and manner; and
        1. must contain the prescribed particulars; and
