178Requirements for application for determination

  1. An application for a determination must—

  2. be made in writing; and
    1. be given, in the prescribed form and manner (if any), to the chief executive; and
      1. contain the prescribed information (if any); and
        1. be accompanied by the prescribed fee (if any).
          1. The applicant must give a copy of the application for a determination to every other party named in, or affected by, the application, either before or immediately after the application is given to the chief executive.
